Our Program

In client-centered care, our programming enhances basic skills for individuals, families (if involved), and communities to promote independence and address various issues like cognitive, physical, behavioral, communication, anger management, safety, conflict resolution, peer relationships, and family matters. Customized programming is tailored to each individual and guardian upon intake.

Your family is important to us, and we are committed to supporting you in fostering positive relationships with them. They are welcomed in our program and encouraged to participate in how we assist and collaborate with you.

If there are concerns about how you express your emotions, we are here to help. Our role is to employ strategies to ensure the safety of both yourself and others while also meeting your needs. We will never take any action intended to harm you, infringe upon your rights, or disrespect you. At times, we may implement safety strategies, such as a plan, but we will do so with your input and that of your family.

We encourage our residents to take responsibility for their finances or to have their family members oversee this aspect. If we need to provide support with finances, we will ensure that all documentation is properly maintained to address any concerns. We will request consent if we are to assist you in paying bills, accessing finances, etc. However, we will not make financial decisions on your behalf.

We will ensure that we practice fire procedures so that you are aware and assured of your safety. Our staff will be well-equipped to guide you to safety in the event of an emergency either within the home or in the community.

Hope Inclusion & Disability Services will support your involvement in the community and facilitate access to activities to maintain your health. We have certain health-related rules within the home, particularly regarding situations where you or another resident may be unwell or have a condition requiring special assistance. Our staff are trained in Universal Precautions and will assist you with strategies to prevent illness and manage infections.

We are here to assist, and there are methods to address any disagreements that may arise, which we refer to as Peacekeeping.

If you or a family member is dissatisfied with the support we are providing, please inform us. If you find it difficult to communicate directly, you may designate someone else to speak on your behalf. We will make every effort to resolve the issue, but if it persists, there are procedures in place to facilitate resolution. These steps are outlined in the Consent Package and detailed in our policies. We would be glad to provide further explanation if needed.

Your personal information is secure with us. We are prohibited from disclosing any information without your consent or that of your designated decision maker. The only circumstances under which we will release information are if legally required, in cases of emergency services, or when there are concerns related to abuse, neglect, or the safety of yourself or others.

How we Help

The Adult Group Living Program adheres to individualized service planning tailored to each individual's needs. Service plans are created within 90 days of intake and reviewed annually thereafter. These plans are informed by formal assessments and observations, integrating client-centered strategies with input from the individual's family, involved professionals, and support network.

Service Team

All individuals are assigned a skilled and knowledgeable Community Support Worker at the time of intake. This role is supported by a supervisor to help guide interventions as directed by the referral source. Additionally, the team consists of the individual or family, the Case Worker, and any vested professionals or significant persons involved.

Request for Services

Hope Inclusion & Disability Services primarily accepts referrals from Disability Services and private guardians. The Director or designated personnel determine the suitability of the placement and ensure that the agency's mandate is supportive.
